I'll go for the whole thing being Caterpillar.

It looks like a yard shuttle, but in that case the wheelbase looks a little long.  The standoff roof is presumably for sun protection, and the only reason I can think of for having the exhaust there is to prevent brushfires.  Not sure if it is a plain exhaust or possibly a spark arrestor too.

Maybe whoever built it really didn't like the driver?

Prototype of the new Cat truck just released built by International ,and initially built only for export , The new version has a few more conveniences including the Cat automatic transmission , I have a few pictures of the new version taken November at the Flying J in Austinberg,Oh . Driver was taking it to upstate NY for Quebec delivery .

Does look like a Cat radiator.

Has a fuel delivery pump on the back.

Re Trak mirrors are the same as on my 1964 GMC. 

Muffler is really pointing in the wrong direction for an aircraft refueller, should have a 90 degree elbow at the back.
